• Spring Hill
• Indooroopilly
• Gaythorne and
• North Lakes (opening Mid 2011)

Should you wish a Referral Pad to be mailed to you, please email: mvalerien@montserrat.com.au or contact your local Montserrat Day Hospital.

As an aid to our call centre staff, please include the patients BMI on the referral.
